PHYSICALS ARE REQUIRED FOR DANCE 3, 4, & 5!
Given that dance requires a physical demand on your body, the school is now requiring members of the DANCE 3, 4, & 5 class to get a physical. This DOES NOT have to be done in order to audition, however, after the class lists are posted, if you are in DANCE 2, 3, 4, & 5, you will have to get a physical done BEFORE dance camp. Here is more information regarding physicals:
- ALL students in Dance 3, 4, AND 5 will need to complete the clearance process.
- Although you will not know which class you are in until after June 12, I just wanted you to have the information, especially since the school is offering a physical day on campus for a low rate of $25 (SEE INFORMATION BELOW!!). My advice? Get a physical just in case!
- All the information and forms are found online at the SVHS website, or in the links below!
- You must complete #1, 2, and 3 on the checklist on the page.
- The driver forms and drug test forms are as needed/voluntary. I will let you know later in the year if you need to fill out the driver form. Right now, the priority is the physical, athletic clearance, and copy of insurance.
- You are welcome to do this through your doctor, as well. A chiropractor cannot complete the physical form.
- I recommend that you read through all the material on the linked page above, as the physical process has changed for our district this year.
- Class lists will be emailed by June 12. If you are in Dance 3, 4, and 5, you will have to have this process completed by August 4 in order to be cleared to participate in Dance Camp and football season.
- If you are required to do this for another sport you are participating in (cheer, STUNT, track, cross country, volleyball, golf, etc.), you only need to do the process ONCE for the whole year. You just need to add dance as your additional sport in home campus. If you have already done this for next year, you are good to go for dance, as well!

PHYSICAL FORMS
The Athletic Clearance form is required for all dancers in Dance 2, 3, 4, and 5. Fill it out before attending the physical day with all your information. The doctor must fill this out at the conclusion of the exam. It has to be a doctor's signature - not a stamp!
The pre-physical day form is only necessary if you are attending the physical day WITHOUT a parent or guardian. The form must be completely filled out before you arrive for the physical day!
